The decision card
Every decision lands in the same six fields
The Decide step ends on a card, not a report. It fits on one screen and says exactly what is being decided.

- What changed, and where the evidence comes from.
- Impact
- What it means for cost, risk, delivery or value.
- Trigger
- The threshold that turns it into a decision.
- Response
- The action, owner and alternative.
- Timeframe
- When it has to happen, and what waiting costs.
- Confidence
- How strong the evidence is, stated as a range.
